Your local pharmacist or physician can provide you with a COVID-19 vaccine.

2. I lost my copy of my vaccination card. Where can I get another one?

You can get a duplicate card by contacting your physician. If you received your vaccination at the Erath County Vaccination Center, you can contact Cary Jackson at emsdirector@co.erath.tx.us
